Center
is a skilled nursing facility (SNF) with 111 beds.
We are Medicare and Medicaid certified.
We offer skilled, rehabilitative, respite, hospice, and long-term care.
Our setting offers semi-private and private rooms.
Our facility is designed all on one level.
We offer three different levels of care at our facility (Light to
moderate care, maximum care, and a special care unit for our residents with
Alzheimer’s and other types of dementia.)
Our daily rates are dependent upon the level of care required. Rates are
available upon request.
Admission
Requirements
We require that all residents obtain a current (within
the last 30 days) History and Physical by their attending physician, indicating
required level of care. Medical
information from a hospital, nursing home stay, or from an assisted living
facility can also be helpful in evaluation of the potential resident.

Our Admission Team will then evaluate the medical information of the potential
resident for appropriateness of placement based upon physical and psychosocial
needs.
